If you were to take the manifold out for the sole purpose of doing the "kegger mod", no, not worth it IMO.
If its out already for the gasket, might as well. Its just a dremel tool and a few more hours of time. Except for the EGR trucks (not sure on those), it will not hurt anything, can only help. On a completely stock truck, it will hurt your low end. If you have made a few modifications, like air cleaner (CAI or 14x3), programmer, Fastman/bored out TB, etc., it will not hurt your low end and will help you on the highway with maintaining speed and accelerating.
