Yes, it can be done. It will be very involved and probably expensive. You can buy a pre-assembled kit (summitracing.com has one) or you can try to peice one together yourself.
Keep in mind that you will need a new master cylinder and proportioning valve (you'll have to lose the factory combination valve) to make disc brakes work properly. So on top of the kit to swap the rear axle parts, you will need those as well. That may also involve completely re-plumbing the rear brakes with all new lines.
