90' Dodge 150 rough vibration

I have a rough vibration while driving at any speed above 20mph. It comes and goes but it feels like I'm driving on washboard roads.

Wheels out of balance, tires out of round, bad shocks, alignment, worn suspension components, any number of things. What have you checked so far?
