Another place was AutoECMs (http://www.autoecms.com). These guys were the most knowledgable that I've spoken with so far. They knew exactly which PCM models would and would not work in my particular truck. They're slightly more expensive at $395 plus $100 core fee, but they also offer and 1-year warranty while I don't think O'Rielly's does.

I'll let you know if I come across anything else.

I wanted to give everyone an update on this. I ordered a new PCM from http://autoecms.com, which arrived yesterday and this has completely fixed my problem. The truck starts up right away every time.
