We are a lot alike. I have an 06 with about 164,000. So far I've done plugs, wires and air intake system cleaner (3 stage) by BG. Recently my alternator and thermostat when out so i had to do that as well. On my 04, i had an intake and superchip flash, but that seemed to rob my gas mileage . I'm thinking my next upgrade for my engine will be an efan.
So if youre looking to get better mileage, get an efan. If you want more power, do an intake and chip. I love the BG engine cleaner, so i wouldnt overlook that due to the mileage your truck has.

My 04 Hemi has 94k (bought with 76k). I haven't done much maintenance to it yet aside oil changes (plugs soon). Put on the K&N intake as its one of the few Cali legal ones. Loved the difference. The Hemi really responded to it. I also ditched the stock muffler and resonator. Had a single chamber Cherry Bomb installed. I can tell she breathes better than before. MPG's havent changed too much. When I baby it i'll make a full 35 gallon tank last a month (I drive 5 miles to work round trip). When I get on the throttle the Hemi roars and will even spin the 33" mud tires real easy but of course the low MPG helps me choose not to.

I would start with some new plugs.I don't know how many miles you have put on it,but after 2 1/2 years you are likely getting close to the 30,000 mile suggested plug change interval.The copper ones are pretty cheap.
A DIY CAI is another cheapish option.The general opinion is that they are about as effective as the ones guys pay hundreds of dollars for.It won't give you huge gains by itself,but it is another addition towards gains.
A "Y" pipe and a freeflowing muffler can be welded into your existing exhaust(assuming its solid) for much less than a catback system, and will help your air flow a bit better.
