Default Down shifting problem

Hi,

I have a 2003 Grand Caravan and I just recently started to experience this problem.

When I go over 60kph then slow down, when I hit 45kph, I could feel the van down shifting but it does not do so smoothly like before. I could feel something like a jolt as soon as the speedometer hits 45kph. This only happens when slowing down from over 60kph. No problems when accelerating. Also no problems when driving under 60kph.

Does anybody know what the problem could be?
