P1391 is intermittent loss of cam or crank sensor signal. Have you changed the crank sensor? Sometimes these turn out to be a cracked flexplate. Before you head there make be sure you thoroughly test and inspect the sensor circuits.

Default camshaft sensor

i replaced the camshaft sensor but later found out they replaced both
cam and crank sensors earlier last year so i am kinda of thinking maybe a wiring issue but not sure both sensors say they are in sync with each other
would a cracked flexplate cause stalling when put into drive but not stall when put into reverse

Yes, it could. Are you hearing any noise from the bell housing when you put the transmission into gear?
