Default HELP....Why are my emission monitors clearing????

I have a 98 Grand Caravan 3.0, Shortly after starting the van I get a slight hesitation or stumble that will almost kill the engine. The check engine light will come on for just a few seconds and then immediately go out. When i check the codes with a scanner i see that all the codes and All the emission monitors have been cleared. I have checked all the engine block grounds and all the battery connection. could a faulty ignition switch cause this problem?

Check ground wire at the battery, We have had loose ground cause that problem. And yes the ignition switch could also cause a problem too. But also check the connection at the PCM.
