Default Overheating and swollen radiator hose

Ive got a 2000 dodge neon, 150k or so miles. My cars been overheating for months. I've replaced the water pump, the thermostat, radiator cap and whatnot. Today it overheated consistently and I was lucky enough to make it home. When I opened my hood I saw that one side of my upper radiator hose was pretty swollen, could've burst not really sure what to do here. Was hoping to get some input.

Replace the hose? It looks like it is still overheating because your coolant is leaking out somewhere. The plastic housing should be looked at and around it.

Looks like the hose needs to be replaced and something is still leaking based on all the coolant in that picture. Are your fans coming on? Any smoke out the exhaust?
