Mystery hole in exhaust manifold - suggestions

Default Mystery hole in exhaust manifold - suggestions

Hello, van is a 1990 b250 with 5.9.

This is strange to me. I've been tracking down a wicked exhaust leak the past couple days and I located it by taping a shop vac hose to the tailpipe, setting the vac to blow, and feeling around for air. I noticed that there is a large hole (like a port) on the front of the drivers side exhaust manifold. Something was definitely there that has fallen off, and I'm taking the van on a long trip next week so I'd like to address it.

My questions are,
1. What was there?
2. How can I plug this?

That or a tube from an air pump system if the van was originally equipped with one.
