Default '99 Transmission Shift problem

My transmission takes time to shift into gear and when it is into gear is doesnt always stay in it. I thought it would be the trans fluid that was low so i checked it and it was so i added some. Then it drove fine for about 5km when i got back into the parking it started slipping out of gear. I let it sit for a while when i went back it worked fine for a few minutes and started slipping out. When it slips and i shift from neutral to Drive i can see the light go from R to D i didnt see if it goes from P since it goes so fast. Any ideas what this could be?

Quick update, when its over 2k rpm it seems to stay in gear... Any ideas?
