Has there ever been any stop leak used in the radiator?

Sounds like a blend door problem.

heater core could be getting old and clogged too.. That might be easier to check then the blend door.. (you might be able to hear the blend door move when you change the temp dealy... i know i can hear it clear as day in my neon)

This may be totally wrong and unrelated, but I once had an old junker that lost coolant. I never figured out where it leaked and to be honest, I didn't want to put money into the car. But the first sign that the coolant was low was that the heater stopped working. The car never overheated because I kept topping it up, but the heater thing was a good sign for me.

Blow out the heater core using a garden hose and the house spicket. Most likely your heater core is plugged up.
