That's likely the pump.
On mine, (96 gasser) the pump shaft broke...... internal to the pump. The pulley stayed in place, and everything *looked* normal. But, when I pulled the belt, the pulley, and a chunk of the shaft just fell out. Explained a lot. (driving it like that, pulling a 4000 pound chipper, was no fun at all.....) I think yours, the pump is just failing, or, it's low on fluid.

You have hydraboost on your truck for the brakes? I don't think it would have anything to do with it.... it just gets pressurized fluid from the pump. The pump is the only thing that does the 'sucking', for lack of a better term. The rest of the system is pretty much sealed, or, it should be..... Is it leaking anywhere??

I would suggest changing the pump, however, last time I did a power steering pump on anything, it took four tries to get one that worked at all..... and even that one was noisy. New rack, new lines, new pump, still noisy. The quality of aftermarket parts has gone down significantly of late.
