Default Engine Failure Analysis

I Oh my engine and 750 miles later, my engine blew #3 rod. I am doing an Engine Failure Analysis so this will not happen again. I have OH my 1999 truck three times with no problems, but this time it failed.

Question, what would cause the new connecting rods with new ARP bolts to come loose at 750 miles?

Improper torque, insufficient bearing/journal clearance, bad luck, moon in the wrong phase when engine was assembled.......

Did the bolts break? Come loose? Post pics??

What do you do to a motor that requires it to be rebuilt 3 times in its short lifespan?
