I haven't thought about this since my 1984 Toyota. Its 4wd shift lever had a screw-on handle, and I bought extensions at the local truck stop. I have no idea whether the Dodge handle simply unscrews like the Toyota, but it's worth a look...
I built a little custom dude at work one slow day. Its a metric rod coupler nut and about 4 inches of threaded rod (M10x1.5 I think), then the handle and orignal nut on top. The blue sleeve is an aluminum tube, one end has a small opening and a set collar to hold it in place. I paint matched it to my truck for the hell of it along with my MAG light on the A-pillar. Not too shabby.
