2001 Caravan Heater Hose Connection help needed

Hello. I have a 2001 Grand Caravan Sport. 3.3 liter. While driving on a cold day I lost my heat and the coolant temp. guage peaked. When I got home, I looked under the hood 4 of my (what I believe is rear and forward heater hoses?) hoses had either burst or rotted off. The problem is in my Haynes manual it tells me nothing about these 4 hoses. I do not know where they go, or what is the easiest way to repair it. They are attached to the firewall...this view is looking aft. They are in a very hard place to get at and taking off the clamps and busted hose pieces was not a pleasant task. I have the hose and clamps to repair it, but have no idea of the configuration. (Also are there more connections that I am not seeing besides these 4?? Any help would be appreciated.

2 = heater core outlet
3 = heater core inlet
4 = to engine outlet nipple
5 = to underbody plumming
6 = to heater return tube and hose

They go to your heater core (2 and 3). The hose that goes into your heater core is comming off your water pump. The hose that is comming out of your heater core is going back into the engine.

Is there a difference in setup between a Grand Caravan with Rear Heat and a Regular (short) Caravan?

The differences are with or without rear heat and with or without oil cooler. Here is the heater hoses without rear heat
