Default Violent shake/shudder

Installed 4" BDS about 16 months ago and kept stock wheels but added 295/60/20 nitto terras (34.4 according to nitto).

Went on road trip recently and noticed several times that truck shook violently when going over uneven road surfaces, especially when one front wheel hits the ridge/hole before the other front wheel. It feels as if I had no control over the truck and was quite scary.

Am going to take it back to the shop that installed the lift but would like to hear from you guys to see what could be causing this? Had the tires balanced halfway into the road trip but didn't improve things.

I am running a half inch spacer and the upper control arm is very close to the tire. Could this be the problem? Perhaps when I hit a hole/ uneven surface at 60-70 mph ((in 2wd of course) the tire hits the upper arm which causes the violent steering feedback?

Do I need to change rims with different offset/backspace to get more clearance to upper arm or could it be a different problem?
