I have the opportunity to restore my late grandfather's 87 W150 with the 318. It either has 80 thousand on it or 180 thousand, but the rear wheel wells are rusted up probably 3+ inches. The steering is shot, and I have no idea how many other little things are wrong with it. It does have a bad carb, but that is the only other thing I know of. My main concern is how bad the rust is. Is it still worth fixing up, or is the old hound a lost cause?

Is it worth it? That's up to you. It is highly likely you will put more money into it than it will ever be worth, so, as an 'investment', it's a bad idea. Now, if you just want to keep it for sentimental value, use it as a truck, or whatever, then fixing it up is a good idea. How far you take that is completely up to you.
