Does the muffler shop usually have to make their own, do I have to purchase separately?

highly doubt it does. I literally just got finished installing a supreme 3" lift on my 95 4x4 and it did not. Nor did I need one.

You could call and ask them. Probably get an answer quicker. There is also a place on the website you link where you can ask a question.

The kit is for a 4wd not a 2wd so no it will not have the steering shaft extension. The 4wd's have an extendable shaft with enough play. The 2wd doesn't extend enough. When my truck was 2wd I did the extension by welding in a spacer on it. The body lift kits will work on a 2wd, just the steering shaft issue. The good kits will include an automatic transmission linkage spacer.
