1998 3.0 runs good cold bad hot

[old as dirt]

starts good and has no hesitation when cold but real hard to start hot and might run on 1 cylinder and then catch and run fine but has a small lag when hot like needs chock on. I have to keep the idle up some at a light or I cant be sure it wont stall and then take 30 seconds to start again. I need help on this one please tom
