Having 2, maybe even 4 2" wide blocks on the bed would be ok, but i would say don't put any on the cab. The bed can be carried by 2 people, the cab is well over 1000 lbs and extra 1" lift would also put a lot more strain on a 2" wide block. Some removable strength loctite on the bolts will make sure the bolts don't back out on you over the years.

Just do whatever it takes to get the shifter **** off. You won't break anything.

There might be a nut under the cap on the shifter. The cap should pop out.

depends on the shifter. my 94 had a nut, the 92 did not and you would ruin it if you tried to get the top off. If the shifter is round and completely plastic, take the cap off and turn the nut. If it is more of a cylinder shape and has a fancy silver sticker with the shift pattern on the top, turn it by hand.
