most likely the reason your book does not show how to replace the inners is that DC does not approve of replacing inner tie rods. Instead they recommend replacing the whole rack which IMO is crazy unless it is damaged or otherwise leaking. As you have found out, there are companies such as Moog, TRW, etc that make the inners and they can be changed.
IIRC, I think there is a special tool to remove them becuase of the way they are designed. I have never had to do one, so I can not advise from experience.
