Default 5.9L serpentine belt riding up on crank pulley

I've replace tensioner, idler pulley and new belt. Belt is riding on front of crank pulley. I've checked the rest of pulley and seems to have no play. What next? Need some help pleased.

Check the pulley/device that feeds the crank for proper alignment.

Yep, the harmonic balancer may be coming apart, I've seen it happen before.
