15x12 is damn wide. It may have to be custom made. Most rims that wide will be taller like min 16in or lager. Why are you trying to find 12in wide rims? If your trying to fit wider tires, just about any size rim even stock can handle anything. I have stock rims which are 15x6 and I have 33x12.5r15 on them. The wide tire still fits. On my Dragstrip TA I have 15in wide slicks on a 10in rim.

i'd say custom made rims... or you could look into the 90's style Viper rims... i think they're like 17 or 18 in.. not sure off the top of my head with the width of them. (i know the rears are wider)
whatever you do... don't run spacers.. i've seen them crack, break, and screw up a TON of stuff because of it... i'll never use them or aluminum drop/raise blocks. (blocks from experience.. they can't handle torque from a stock 3.9L)
the 6x114.3mm (or 6x4.45) is a Ba$tard lug pattern. or what you could do is snag parts off a 1st gen dakota... that way you'd be able to use the 5 lug setup. that swap isn't uncommon to do to these daks anyways.
