Did you replace it with a used trans, and MORE importantly, have you checked the wiring harness?

I just checked the code for this and it comes up as "governor pressure sensor voltage too high/too low".
Trans failure (which you seemed to address already)
Faulty gov pressure sensor
Gov pressure sensor harness open OR shorted
Gov pressure sensor circuit poor electrical connection

I'd install a kit in it.
It has sensors,governor,

an that spring that breaks a lot in these trannys.pic may differ but similar.
My spring was in 3 pieces an effected the pressure for shifting..I adjusted the bands too.
Yours may be just the sensor but check that spring!

+1 on hidden's post regarding the broken spring. Happens a LOT! Also for the governor pressure solenoid as it will definitely help shifting by controlling line pressure in the trans. Still, don't be afraid to check your trans wiring harness for broken/frayed or burned wires. I cannot stress this enough as this is overlooked as much if not more than other things associated with Dodge tranny issues.
