Default O2 Sensor

We have a 2000 Dakota & all of sudden it has been a mess. The fuel modulator pump had to be replaced. After that, the O2 sensor went out. So far it has been replaced 3 times in the last month. Got it back Friday from having another one put on, didn't get 5 miles, check engine light again! Does anyone else have this problem? What is the deal?

Here it is:

Can you tell it has been eating gas? At it's best it was doing a little over 3.5 mpg.
