Default can a lug nut lose its cap?

I noticed after taking my truck into the dealership for warranty work that a strange lug nut was present on a tire that had been taken off and put back on by them.

It was on the tire, but it was just a nut, or more like a small do-nut. There was no doubt in my mind that the mechanic had just put the wrong nut on.

However, the service advisor asked first if I had always had it. No, not the case. Then he told me the cap had just come off the nut because it had been used too many times.

Is this possible? I have standard type lug nuts on my truck - the alloy-looking ones, not chromed.

I lost 3 covers so far off of my 2000 Dak QC. ****ty design. Garage put new sneakers on, then one day when I went to rotate them there where a few lugs I could not remove because of their impact guns. So as I was removing them with the lug wrench they all stripped. I ended up using one of those nut removing sockets and they finally came off......the cheap covers..... that is. Low and behold, there was a nut under there! Have fun getting one off if you strip it.

I pulled all the caps off mine and put chrome lugs on,
(i'm gonna replace them with red anodized ones)
they are just a hassle for me to deal with, their only function is to hide the actual lug nuts
