Trying to change my rear drum brakes to discs due to right rear wheel cylinder rupturing every year. never tried anything like this. wondering if anyone else has and if it is possible.

Rear disc swap is very easy to do. You will need the brake lines from the rear disc lines. All read disc brakes are the same from 95-05.

X2^ I did it this way bolted right up, didnt have to change anything just bolted on and bleed the system, and away I went, course I had to order the E-brake cable due to it being cut when they were unbolted. If you go this route check the spindle, mine was bent, but the one from my sxt crossed right over.
