Default Lower Idle problem?

Okay everyone its been a while since ive been on but im back!

okay so lately ive had a low idle in drive and park.
my normal idle is between 600-750 rpms in park and right now its a little above 500

and in drive its under 500

also i have hard starts where sometime my truck will start right up and jump to about 2000 rpms and and then just either fall back down to too low of rpms to stay running or it will catch itself at around 100 rpms and go to regular idle. Im not quite sure what the problem is so heres what ive done so far to eliminate any possibilities u guys come up with:
New Fuel Pump
New PCV
New Breather valve
vacuum lines
Lucas fuel treatments
I run only 91 Octane
110,000 miles

thanks for the help!

I like to think I take very good care of my truck, just like everyone else on this forum, but just for kicks I decided to take my throttle body off and clean it. As if it needed it, so I thought. OH CRAP, did it ever!!! The IAC tip had a thick coat of I don't know what, maybe dirt? Well, whatever it was I cleaned it down good with rubbing alcohol and Q-tips. Now it idles even smoother then before. I suggest you take a look at that REGARDLESS of how clean you THINK it is. The only thing it'll cost you is time. Food for thought...

I can't really speak on that...the O2 sensor. I've had mine on my truck for about 5 years now with no problem. NGK O2 sensor.
