Default Transmission upshift issues

I have a 2001 Ram 2500, 5.9l Cummins w/ auto trans.
The transmission will not shift out of 1st unless you let off the throttle. Once you reach 30 mph the trans seems fine. I have changed the fluid and replaced the shift soleniods but this has not changed anything.
Is ther anything else I can try before taking it to a trans shop.
Any help would be greatly appreciated.

I tried to adjust the front band however the adjusting screw threaded all the way in by hand.
I assume this means the adjuster is broken. Can the adjuster be purchased or is my tranny toast?
What harm will come if I drive it like this?
Thanks for all your help.

You can buy that part by itself HOWEVER before you do that, ask MonteC what he thinks in his thread. He's our tranny expert.
