I recently noticed when i tried to grease my universals that the rear one is installed backwards. They did this shortly after I bought the truck and it has still made a clunk in the drive train even after they replaced them. Could the fact it is in backwards make it clunk?
It only has one place for a fitting and they put it in the tight bend if the u-joint was removed and turned 180 deg it would match the one in the rear as far as location and i could grease it with ease

Just raise the vehicle (using all safety precautions of course). Put it in neutral and rotate the shaft to a position were you can get at the fitting!
And beside the fact that they can't be put in backward as VW has stated, your problem is with the diff. Which is pretty normal for a RAM, or if it's real bad, the diff is on it's way out.
