Definitely is a ext cab, either that or it has a super long single cab. lol

All 2nd gen 3500's were duallies.

The pre 97 black mirrors mean black bumper is wrong as I have a '94 with black mirrors (they are black plastic not painted) and my bumper and grill are chromed with no indications that they were ever changed. I have actually never seen a factory black painted bumper on a dodge on anything but a sport model.
