i had this problem and it was a cracked flange where the transfer case and the trans tail housing met i had to replace the tail housing ( thank god for pull a part)

 

either that or you got a bad track bar had two issues at different times cause the same problem well almost track bar more front end shacking cracked flange was more in the cab right below you kinda shacking. only when i would get up past 45 50 mph
