Won't idle until warm, or after long periods of sitting

The truck is easy to start when cold, however, it does not idle independently. In order me to get the truck moving I have to slightly press the accelerator and kick up the RPMs. After a 1/4 mile of driving it will idle fine. What might be causing this problem?
It has gotten worse over time. At first, when the truck first cold-started, the RPMs would pulse every second from 400-600 or so and occasionally dip low enough for the truck to cut-out. It slowly got worse. Eventually it was dying more frequently until it would not cold-start and idle at all.
Any suggestions?

Try cleaning the IAC. Grab some sensor-safe (e.g. MAF) cleaner, remove the IAC (from the rear of the throttle body), clean it and the bore it came out of nicely, stuff it all back together. Remove the negative battery cable first, and reinstall it when done so the PCM will have an excuse to figure out its world when it wakes up. Oh, and don't push/pull/twist the pintle. Just leave it right where it is.
If at first it doesn't appear to have worked, get to the nearest place where you can legally and safely maintain a 40MPH cruise for about half a minute. At some point during that half minute the PCM will (try to) calibrate the IAC.
