Make sure the tensioner is free and not froze up.

check that the belt is properly lined up on each pulley. sometimes they'll slip on one of them by one rib.

with engine running and your hands completely out of the way, look and see if the belt is tracking straight.

unhook the belt from the tensioner.
rotate and wiggle each pulley by hand. check for silent spin and no wobble or clicky. make sure tensioner spring is working correct to keep the belt tight.

remove belt and check its condition. should be no threads showing through, no cracks,

if nothing obvious, spray the pulleys with brake cleaner to clean them of mud and oil. let dry and reinstall everything.
