Default Valve clatter and P0171 code

2001 5.9L, Ram 1500 4x4, 200k miles
I've been having some loss of power and some valve clatter when I step on the gas at highway speed. Today I got the too lean, bank 1, sensor 1 code. I changed the O2 sensors about 5 years ago (maybe 50k miles). I think I put in NTK sensors. I've read a bad O2 sensor can cause this code. Should I start with replacing the upstream sensor or is it likely still good?
How long do these trucks go before before they need a valve job?

I'm starting to think it my be either the intake gasket or maybe the plenum. Would a leaky intake just affect bank 1, sensor 1?

Unless you have a California truck with 2 upstream O2 sensors, there is only one upstream sensor bank 1, sensor 1. If you think you have a leaking intake that would be the place to start. Not sure if it would cause a lean mixture since the fuel is port injection.
