Exhaust on an 03 1500 4X4
I get like 10-12mpgs in town and 17-21mpg highway
The city would be better but I'm a very aggressive driver and usually going 10-15 mph over speed limit not to mention I love to mess with the little rice burners

Add a CAI and an E-Fan an that will help improve it a little bit and also add a few HP but nothing to be amazing but I guess every little bit counts
With my CAI I didn't see improvement in gas but I did see improvement in throttle response an maybe a little more get up and go

You're doing great with the 5.9 V8. I had one in my '98 RCSB 4x4 and never could do better'n single digits in town and might've nipped 12 on the highway IF I was real easy on it. Now granted I had 5" of lift and ran 35" tires BUT my '04 is a QC Hemi and does about 13 in town, 17'ish on the interstate with 4" of lift and 35" tires with pretty much the same driving habits.

Your on the low side of normal if you have a 4x4 and aren't doing well if it's a 4x2. I find 4x4 QC 4.7s to average about what the Hemi does which on average would be about 13 in town and 18 highway. Could very well be a proper tune-up is all it needs but you could also check your tire pressure. For everyday driving you should keep 'em at about 80% of the MAX cold PSI listen on the tire. Running tires just 10 PSI low can cost you 3 MPG easy...

Whats the best way to do a CAI? Buy one or make one?
Hammer:
I just did a tune up and rotated the tires. I check my air pressure every time I fill up. Maybe I need to get an injector cleaner and run through it.

Try buying non-ethanol gas if you can find it, if not pick up a bottle of Star Tron (I get mine in Wal Mart - outboard/boat section). It's only about $7 and change and treats 128 gallons of gas. It negates the effects of ethanol, stabilizes/revitalizes fuel and cleans the entire system including injectors. I regularly see 2+ MPG better when I use it...
heres a link for gas stations that sell non-ethanaol gas:

That will tell you how to do it but fitting for your 4.7 will be different because that guy did it on a Hemi
How I did it was use the stock cap on throttle body and took stock air box off and then bought a CAI pipe from auto parts store and put it into the flex part of the stock cap and then clamped it on and added the aftermarket filter and bam it works but I believe the stock cap is a little restrictive so if you did use PVC pipe it might be a little better but not by much and it cost me about $45 for pipe and filter
Hammer: I get 10-12 on a good day when I do the instant Eco as I drive I'm only getting 7-8mpgs and as for the highway I average more 17-18 which people tell me is awesome for my truck but I have seen it up to 18-21 a few times
