Default 2002 dodge ram 4x4 brake failure?

i have a 2002 dodge ram 2500 4x4 cummins diesel.
i was recently in a accident where my front brakes were completely gone and my rear brakes locked up causing me to literally leave marks while sliding into someone. yeah sucks!!!
my question is if anyone else has had this problem? or rather is this a occurring issue in this truck im not aware of?
what could possibly cause this?

When you change your brake pads, do you just compress the caliper, or do you loosen the bleeder when replacing your pads? If you don't open the bleeder, you risk pushing dirt backwards through the line up into the ABS system and master cylinder. This has happened on me in a Honda Civic (without the accident). It was due to the caliper locking up while braking. It also started leaking when it happened causing the rest of the brakes to fail to work. Since it was both rear tires tho, I would be concerned about it maybe being the parking break failing somehow first. Then I would check out the rest of the brake system for any damage or leaks.
