The fact that it just suddenly started running super rich suggests that the fuel pressure regulator may have taken a dump too . typically, if a sensor fails or goes out of tolerance, a code will be set. From what I'm reading, he only code you are getting is the too rich code, which leads me to believe it is something other than a bad or suspect sensor. Check the basic stuff first and start eliminating possibilities.

Well, I've been doing some more reading and I'm starting to wonder if one of the valve springs might have broken... sounds like its a somewhat common problem with these engines...

Stinky exhaust and wont idle/run properly sounds like the start of a plugged/bad Cat.
This can happen from bad injectors that pour excessive fuel. Unburnt fuel gets caught in the cat where it burns and melts the henoeycomb insert pluggig it up.

Hmm, before I take a sawzall to the cat, any way to test? Could I pull the upstream O2 sensor (but leave it plugged in to the harness) and try to run the engine and see if it runs a bit better when some exhaust pressure could escape out of that hole where the sensor normally goes?

I would think so on pulling the sensor, but you may want to some how keep the outer housing connected to ground for reference.

Also look to see if both are carboned up. I assume they would be given its running rich.
When you do have them pulled, check the voltages again, I would also assume with them pulled, they should be reading nearly the same give/take a little.

he did get the voltages, they are very high which would indicate it is not a plugged cat, because it would read lean due to the lack of exhaust getting passed it.

Check for vacuum leaks too.

Here is a little video of it running. Now that I watch the video and listen to the way the engine turns over, I'm really starting to think maybe there is a broken valve spring - sounds like one cylinder doesn't have compression. What do ya'll think?

I read this one post here and it sounds exactly like the problem this engine is having.

Hi. Does the engine run REALLY bad, like it has a high duration race cam in it? I know it sounds stupid but if is just stumbling or running slightly rough it could be a lot of things. If it is running really bad and you are hardly able do drive it, I think you may have a broken valve spring. When the valve spring breaks, it allows the valve to drop slightly and cause some turbulence in the intake which screws up the MAP sensor readings causing it to run like complete crap. I have had to repair many of these for this problem and if you drive it too long, you will end up with a valve that looks like an S after it is sucked into the cylinder and smashed. 2003 trucks are the only year I have seen this happen on. There are revised valve springs available and I recommend replacing all of them if this is the problem, actually i would recommend replacing them on any 2003 hemi just to be safe. If it gets too bad you will be replacing the engine. Get it checked out or pull off the valve covers yourself, its not as hard as it looks. This is really the only fault with these engines and it was fixed by 2004. Hope this helps.
At this point I'll probably throw it on my trailer and take it to a shop - its not my truck and I just don't have the time or all the necessary tools to go through everything it could possibly be, and at the same time I don't want to keep it and risk having the valve get sucked in and hit the piston and cause a lot of damage. The old Cummins will probably enjoy getting to tow something anyway, its been a while...

I think you are on the right track taking it to the shop, definitely dropped a cylinder-----
almost tinney sound. I wouldn't run it anymore for fear of more damage.

This one might hurt a little in the repair.

Well curiosity got the best of me so I went ahead and pulled the valve covers and look at what I found, cylinder #4 intake valve spring looks like its toast.
