Adding a 90mm throttle body
Hey, I just did this over the weekend, but not on the Hemi.... My first test I ended up blowing out a header gasket.... Not the TB fault obviously.
I have thought about this for the Hemi, but to be honest I am done modding it. It is my DD, I have done a couple little things and am happy with what I have.

Yeah, anything over an 85mm will require a proper tune to reap the benefits of "any" performance gains at WOT. As much as I've done with my Charger, I'm still running the stock TB, but have thought about upgrading to a 85 eventually.
And who are these "people" who maybe get 5 hp?
Do you have a source for those numbers or did you just make that up?
The BBK website advertises a gain of 10 to 15 HP, other sites such as shophemi.com shows about the same.

Most manufactures as well as distributers such as HHP make that claim on their website. I have other resources as well locally who own dyno shop and have done quite a bit of work tuning my 5.7 over the years that would agree.
Why wouldn't you have your ECM recalibrated, if ment the difference of 5-10 rwhp?

I don't know that 85mm is the magic number, but I do know that modern ECMs are programmed to operate within a designated range of data (intake temp, velocity, etc). The stock ECM will attempt to make adjustments to fuel, spark, and timing, but once you start feeding data too far out of specs, it will throw a code. This is where a "flash" or "reprogram" becomes necessary.
Some vehicles will actually advance timing to keep values "stock" when you increase airflow or other other such things. The darn things are too smart for their own good!

I don't agree, Every little bit is helps, If bottle necking is really an issue than everyone who installs a CAI, is wasteing there time, and the same to those who install freeflow exhaust systems, being the exhaust alone is not alowing for more air into the engine. Sure it would be nice to go with CAI, larger throttle body, upgraded intake manifold, headers, true dual free flow exhaust.
In defence of RTCarlin... I believe he was comparing a stock engine with a throttle body spacer. A CAI "helps" with increase forced air, a true dual "helps" with less back pressure, a throttle body spacer (by itself) is really no more then an extra extension on your stock intake, with the same ID/OD bolted to you intake manifold.
