Default Weird or what?

So I had a weird thing happen today, I don't know if I should be afraid or not...

When I was just pulling out of town, my truck surged forward. Didn't really clue in till a little while after when my RPMS were too high. The truck had shifted into fourth and locked up! it would not shift into 5th or OD, but would into the lower gears...

I pulled over shut the truck off restarted it, worked fine afterwords...Didn't throw any codes or nothing...

Don't know if I should be worried or not, any clues as to what it is?

You may have inadvertently hit the manual shifter and downshifted to 4th, restarting would cancel your action and it would then shift automatically.

I've accidentally downshifted at stoplights several times by bumping the gearshift. At least I assume I did because when I took off and it didn't shift up I'd find it locked in a lower gear.

In my 2012, I noticed something similar, but it only happened once. I was pulling up to an ATM (drive-thru). I had my foot on the brake, slowing down. The truck surged forward and it took considerable braking pressure to make it stop. It felt like all the articles a couple of years ago describing "unintended acceleration" in toyotas. Haven't experienced anything like that since.

It was not an accidental thing...because I tried afterword to shift it back into OD (holding to the right) The Evic went back to normal at that time, but 2 seconds later it showed a four again and there was not shifting during this time...
