It's by a company called Alpha Therm. Model AT-380Di. Neat little gadget. My buddy has an Escalade with heated washer from the factory and it makes short work of frosted windshields. Saw it last winter and decided to check on something for my truck. Found this. Costs about $100.

Installation is really easy on the RAM. The washer fluid hose running into the hood is easily accessible just behind the battery. There is a good spot to mount the unit on the top of the firewall. You can lift up the black plastic cover surrounding the wiper arms there by removing a couple darts and get behind for putting nuts on the mounting bolts (or you could use sheet metal screws). Battery hookup is stupid simple there. The model I listed has a remote switch that can turn it on or off, which is the topic of this thread. There is another model (see below) that powers on whenever the vehicle is running. The unit just heats fluid as it passes through.

That one doesn't have the switch capability. It just turns on when input voltage is over 13V (i.e. when the vehicle is running, because battery puts out 12.x when not running, and 14.x when running).
