1995 caravan. fuel gauge says empty but has half tank

1995 caravan. fuel gauge says empty but has half tank

The fuel guage keeps saying its empty gas light comes on. When I put gas it does say how much I have but while your driving gauge is going down till light comes on.

the tank say it is empty, but it is not. what would that be or what do i need to change on it?
