Default 92 T&C Transmission Troubles

Not related to the wiper issue I also just posted...

1992 Town and Country, 2WD Automatic 3.3L.

I had the transmission rebuilt a few years back. My warranty has expired (probably about 12 months and 12,000 miles ago).

It never really shifted smoothly after being rebuilt, but was livable. It has gotten worse as of late, with the following symptoms:

-Always rough shifts in lower gears (mainly 1st to 2nd and back)
-Occasionally (a handful of times over the past 6 months) won't shift in to higher gears, forcing me to pull over, and restart the vehicle
-More recently, delays in shifting from park or reverse to drive, where occasionally I will start to go backwards before it finally engages to go forward.
-Similarly, if shifted into drive right away from starting, but not allowed to drive (ie, foot on the brake) it may repeatedly engage and disengage while the brake is applied

My fluid levels look okay, and the fluid is clean.

Are there any sort of vacuum hoses or other small things I can look at to figure out what's up with it?

My Haynes manual says of the throttle shift cable: "If the adjustment is too long, the transaxle will shift early. If the adjustment is too short, it will shift late."

It says the throttle cable "controls a valve in the transaxle which governs the shift quality and speed. If shifting is harsh or erratic, the throttle cable should be adjusted."

I assume this (these?) would be a good thing to check as well?

It is apparently going in to limp mode when this happens.

It seems to happen sometimes after a long drive that heats the engine up and then parking for a short while, and then trying to drive again afterwords.

If I pull over and restart the van, it usually clears right up.

Tried to see if Advance Auto could do a code scan, but they can only do 1996 and newer. The key on-off-on-off-on code check does not bring up anything.
