I have check engine light on, so i took it to autozone and thay gave me these codes

Idk what it could be,

But thw minivan runs fine,
And when i came home i stayed home all day and today in the morning i tried to go out and it wouldnt start like if it wasnt receiving gas so i pressed on the gas and cranked it up and it cranked up, then after 4mins it started right up, i also checked and found some kind of smoke in the floor which is dirt floor so it was like a powder, its black, every time it starts it spits that powdee thing, also when its on and i accelerate when it reaches like 5rpm's
It like jumps acelerating i dnt know how to explain it, it like skips while acelerating
