What year/trim level is the van? It helps to know if the car has a mechanical throttle cable or electronic throttle.

Looks like that year has a mechanical throttle cable. It's hard to tell with the details provided, but it could be as simple as slack in the throttle cable, or your throttle plate or intake manifold are dirty. It could also be something more complicated like a problem with the PCM.
Another good data point would be if it does this only while the car is in gear, or does it also do it when the car is in park or neutral?
