[4th Gen : 01-07]: High RPM Oscillations

I have a 2002 Grand Caravan Sport, has 113k miles.
I recently changed the transaxle fluid and filter. I was very careful to use the Chrysler recommended fluid (ATF+4) and get the level correct, and all that looks good.
Not long after that I've begun noticing an oscillation in RPM at high engine loads and high RPM. We have mountainous terrain, and on one steep incline I was doing about 50 mph @ 5000 RPM when the tachometer began to oscillate about +/-250 RPM around the 5000 RPM mark right after shifting from 3rd down to 2nd. I've also noticed at freeway speeds when climbing inclines that the transmission will hesitate when shifting from 3rd gear back up to 4th after climbing said hill.
I've searched Google like I normally do when I have issues with our caravan, but in this case haven't been able to find any info. I'm hoping this isn't a symptom of a serious problem.
