Unhappy Transmission Slip on 2014 Caravan

Hello Folks,

I have been having transmission issues the day I bought this van (brand new) back in 2014.
I currently have 41,000 miles on it.

1. The first issue was the van would jitter when backing up on an uphill surface such as a driver way. (It feels like driving a stick and its about to stall. Took it 3 months after I bought it and was told this is normal with these vans

2. 2 Years later (before the limited warranty ran out) I took it to another dealer, and he took a look and noticed the issues. He had to pull the transmission out as no codes were firing and was determined that some blades are worn and had to be replaced.

3. 3 months after this job, when I am on the freeway, when switching from 4th to 5th gear, the tachometer seems to jump 300 rpm before it goes down. Same happens when going from 5th to 6th Gear. Also seems to be a lag when up shifting. Dealer is unable to "replicate" and said no codes are firing and said there are no issues.
