Hi I have a 1996 Dodge Grand Caravan 3.8L. Today when I step on the brake, a scratching metal sound occur. There is sometimes a scratch metal sound while driving too. Does that mean my brake pad is wearing out? Thanks!

If the pad thickness is 1/4 or less it's time to change the pad, especially if it is rivited, as yours are. The noise you hear may be the wear indicators scraping the rotor, another indication it's pad replacement time. Do both sides it won't take 1/2 hour to do the job.
