Default Replacement Maint book?

Does any one know where I can buy just the skinny maint book? I had mine lifted out of my truck years ago. It had reciepts and stuff stapled into it so it was taken...
I looked a while back online and I ordered a whole new Owners manual set but it did not include the little scheduled maint book. Any help here? A few other posts reminded me about my 5 year inspection coming up and looks like I gotta do the coolant and diff before July. Only 39k on my 09 but better safe than sorry. Thx fellas.

I guess know one knows. The only source I could think of is a dealer. Other then that try ebay. Not exactly a sought after item apparently. I don't know if I even still have mine.

I'm not sure I've seen the book you speak of...

I use a FilemkerPro database to track the maintenance/repairs on all my vehicles.

I used a worksheet to input/track the maintenance records of my rig. A bit tedious to do but I want something more organized.
